Kun ihminen lihoo, hänen ihonsa venyy. Lihomisen myötä venynyt iho ei välttämättä palaudu entiselleen painon laskiessa. Ylimääräinen iho muodostaa poimuja. Poimut voivat olla pieniä ja miltei huomaamattomia, mutta suuren painonpudotuksen jälkeen suuretkin ihopoimut ovat tavanomaisia.
Poimuttunut iho on useimmiten esteettinen vaiva. Joskus suuri ihoylimäärä aiheuttaa kuitenkin myös terveydellistä haittaa. Kasvojen alueella ihon veltostuminen on usein melko lievää, mutta vartalolla venynyt iho saattaa pussittaa paljonkin. Useimmiten suurimmat ihopoimut muodostuvat seuraaville alueille:
Teen viikoittain useita operaatioita, joissa poistetaan ylimääräistä ihoa paljon laihduttaneelta. Näiden leikkausten määrä on kasvanut myös siksi, ettei julkisella puolella ole enää rahaa hoitaa niitä. Usein roikkuvan ihon terveyshaittoja ei katsota riittävän suuriksi.

Painonpudotuksen jälkeen tehtävä kirurgia (postbariatrinen kirurgia) korjaa laihdutuksen myötä syntyneitä ihopoimuja. Leikkaukset voidaan tehdä siinä vaiheessa, kun riittävän alhainen paino on pysynyt vakaana noin vuoden ajan.
Ennen leikkausta potilaan painoa seurataan. Riittävän alhainen paino on, jos painoindeksi on alle 32. Leikkauspotilaan tulee olla perusterve sekä fyysisesti että psyykkisesti.
Painonpudotuksen jälkeen ajankohtaisia toimenpiteitä ovat esimerkiksi rintaleikkaus, rasvaimu, abdominoplastia (vatsanpeitteiden kiristys) ja käsivarsien muotoiluleikkaus. Sirkumferentti abdominoplastia (bodylift) on myös yksi vaihtoehto silloin, kun potilas on pudottanut paljon painoa ja iho pussittaa huomattavasti. Tällöin vatsanpeitteiden kiristyksen lisäksi kohotetaan myös reisien, selän ja pakaroiden kudoksia, ja leikkausviilto tehdään “sirkulaarisesti” koko alavartalon ympäri.
Painonpudotuksen jälkeen alavatsalle saattaa muodostua poimu, joka roikkuu intiimialueen päällä. Poimu saattaa haitata peseytymistä ja henkilökohtaisesta hygieniasta huolehtimista.
Vatsanpeitteiden korjausleikkauksessa eli abdominoplastiassa poistetaan ihoylimäärää sekä kiristetään ihoa. Usein myös lihakset korjataan oikeaan asentoon.
Joskus ihopoimu ympäröi koko kehon jatkuen alavatsalta myös selän puolelle. Tällöin voidaan tehdä ns. sirkumferentti abdominoplastia eli bodylift.
Laihtumisen jälkeen sekä miehen että naisen rinnat laskeutuvat ja alkavat pussittaa. Jos poimu on suuri, poimunalusiho saattaa hiertyä rikki tai jopa tulehtua. Tällaisessa tilanteessa rintojen korjausleikkaus on lähestulkoon ainoa keino korjata toiminnallista haittaa.
Kun paino putoaa useita kymmeniä kiloja, myös olka- ja käsivarsien iho saattaa löystyä huomattavasti. Yleensä pussitus käsissä on maltillisempaa kuin vartalon muilla alueilla ja vaiva ainoastaan esteettinen. Esteettinen ongelma voidaan hoitaa olka- ja käsivarsien kohotusleikkauksen avulla.
Reisien alueella poimu muodostuu tavallisimmin aivan reiden tyviosaan. Tämän alueen poimut aiheuttavat melko usein ihon hiertymistä, haavaumia ja tulehduksiakin. Ihopoimu voidaan poistaa sisäreisien kiristys- ja kohotusleikkauksen avulla. Samalla voidaan poistaa alueelle jäänyttä ylimääräistä rasvaa rasvaimun keinoin.
