What the surgery is like?

Eyelid surgery is a relatively straightforward procedure, especially when compared to other cosmetic surgeries. The surgery can be performed as an outpatient procedure under local anesthesia.

Before the surgery, the operating surgeon creates a so-called surgical map of the area, which helps in removing excess tissue. The actual surgery is performed with a scalpel or laser. At City Clinic, the use of an electric scalpel is preferred.

The surgery itself typically takes about two hours. Often, the patient stays at the clinic for some time for observation before going home.

 

 

Laser or scalpel?

Eyelid surgery can be performed using various tools such as a scalpel, laser, or electric scalpel. The choice of instrument does not affect the outcome of the surgery. Rather, the result depends more on the skills of the surgeon using the surgical tool.

Some surgeons prefer using lasers, while others prefer performing the surgery with a scalpel. It primarily reflects the personal preferences of the surgeons and, to some extent, differences in their training.

If the surgeon is skilled, has extensive experience, and a strong aesthetic sense, the surgical outcome will be equally good whether a laser or scalpel is chosen as the tool.

Who performs upper and lower eyelid surgery?

It’s important to keep in mind that the result will be an improvement, not necessarily perfection. One of the most important aspects to consider when planning eyelid surgery is your mental well-being. Eyelid surgery can enhance appearance and boost self-esteem, but everything else depends on the individual.

In addition to traditional eyelid surgery, Cityklinikka also performs Asian eyelid surgery, in which Dr. Akmaral Karakulova creates an upper eyelid fold for the patient. Asian eyelid surgeries are primarily performed for people of Asian descent.


Upper eyelid surgery

Upper eyelids may droop for various reasons. Some children are born with this condition because the muscles that lift the eyelids have not developed normally. These children may need to tilt their heads to see properly. In such cases, a specialist should be consulted, as drooping eyelids can be associated with other issues.

Most often, drooping eyelids are caused by aging. As we age, all tissues lose elasticity, including the tendon that connects the eyelid to the muscle. The skin of the eyelid stretches over time. Since eyelid skin is the thinnest on the body, it’s usually the first to lose elasticity. Loose upper eyelid skin can impair vision and give a heavy, tired appearance.

Lower eyelid surgery

Puffy lower eyelids can make the whole face look older and give a tired appearance. If the look of the lower eyelids bothers the individual, surgery may help. For some patients, puffiness is due only to loose skin.

Often, there is also excess fat in addition to loose skin. The procedure is always tailored individually based on what causes the puffiness and how extensive the issue is. Lower eyelid surgery is often combined with fat transfer, which rejuvenates and brightens the midface area.

In some cases, an effective laser treatment series may be an alternative to surgery. However, in some situations, only surgery can provide a lasting solution.

Lower eyelid surgery and other options

Common problems of the lower eyelids include excess skin and so-called eye bags. Thin skin may also cause issues. Especially with aging, thin skin becomes dry and wrinkled. Lower eyelid surgery offers a permanent solution to these cosmetic problems.

Sometimes the appearance of the lower eyelids can be improved without surgery. Non-surgical procedures mainly tighten loose skin. If there is a lot of excess fat, non-surgical options may not produce the desired results.
Non-surgical treatments suitable for the lower eyelid area include:

Eyelid surgery procedure

Several surgical techniques can be used in eyelid surgery to restore the eyelid to a normal position. For example, the muscles that open the eyelid can be shortened. Excess upper eyelid skin may be removed. Removing excess skin from the upper or lower eyelids can improve both appearance and the field of vision. If there is a fat hernia in the eyelid, it can be removed at the same time, making the results longer-lasting.

Upper eyelid surgery can be performed with a surgical blade or laser. In both methods, excess skin and fat are removed from the upper eyelid. Sometimes the muscle is also shortened. At Cityklinikka, eyelid surgeries are performed with an electric scalpel to avoid burns to the tissue, which can occur with laser procedures.

Lower eyelid surgery can also be done either with a blade or a laser. Laser surgery is almost bloodless, but it may cause tissue burns, which can prolong recovery.

Puffy lower eyelids can sometimes be treated with a series of Smile Eye Lift procedures.

Eyelid surgeries are usually performed under local anesthesia. For both upper and lower eyelid procedures, the scar is almost invisible. On the upper eyelid, the incision is made in the eyelid crease; on the lower lid, it’s along the lash line.
Come to the procedure without makeup, jewelry, or nail polish (fingers and toes). Eyelash extensions and piercings must be removed beforehand.

Recovery

Healing after the procedure is individual. Swelling and bruising may last 2–3 weeks. The skin around the eyes may remain numb for about three months. Makeup can be used on the eyelids about 7–10 days after the procedure. Eyelids must be well protected from the sun. Stitches are usually removed within a week, and the wound generally heals to an almost invisible scar within a few months. Complications are extremely rare. Scars are usually completely hidden.

  • After surgery, eyes are cooled with ice packs for about an hour.
  • Cold compresses should be continued at home for the next two days.
  • Eyes should be rinsed daily with cool water.
  • Physical exertion should be avoided for 2–3 days after surgery.
  • About a week after surgery, you may wear makeup again. We recommend using neutral (e.g., fragrance-free) cosmetic products. Protecting the skin from sun exposure is important during recovery.
  • Bruising, swelling, and scars can be treated with effective Arnizen™ cream.
  • For swelling prevention and treatment, lymphatic therapy is recommended.
